Ketogenic diet and mood disorders: insights from a narrative review
A recent narrative review examined the emerging evidence for ketogenic dietary interventions as adjunctive therapy in mood disorders. The authors note that randomized and prospective studies suggest benefit for depressive symptoms, but methodological heterogeneity and safety concerns limit current clinical recommendations.
Study and findings
The authors conducted a narrative review of recent literature on ketogenic dietary therapy in mood disorders. They highlight that several randomized and prospective trials have reported improvements in depressive symptom scores when the diet is added to standard treatment. Adverse events were more frequently reported in participants of normal body weight or in those lacking proper nutritional guidance. The review also points out the absence of uniform criteria for defining the ketogenic protocol across studies, which hampers direct comparison.
Clinical interpretation
The reported improvements suggest that a metabolic shift induced by a ketogenic regimen may influence neurobiological pathways implicated in depression, such as mitochondrial function and neurotransmitter balance. However, the evidence remains limited to short‑term outcomes in selected trial populations; the magnitude of effect and its durability are not established. Safety considerations, particularly the risk of nutrient deficiencies or dyslipidemia in individuals without excess weight, must be weighed before adopting the diet in routine psychiatric care.
Limitations and open questions
The review underscores three major gaps: (1) the small number of adequately powered randomized controlled trials, (2) the lack of standardized ketogenic protocols (energy intake, macronutrient ratios, duration), and (3) insufficient data on long‑term efficacy and safety in diverse mood‑disorder phenotypes. Future research should employ uniform dietary definitions, include larger and more heterogeneous cohorts, and assess both clinical and metabolic endpoints over extended follow‑up.
